What this template is for
Use it for letters that need to look official: to a landlord, a bank, a government office, a supplier you are leaving, or a customer about a change in terms. Many banks and offices still ask for a letter on company letterhead.
Fill in the letterhead once and keep the file. Every letter after that starts from your own letterhead.

Questions
Yours sincerely or yours faithfully?
Use "Yours sincerely" when you know the person's name, and "Yours faithfully" after "Dear Sir or Madam". In the US, "Sincerely" is the usual close.
Do I need a subject line?
It helps. Offices that get many letters file them by subject, and it tells the reader what the letter is about before they start.
Can I add my logo?
Yes. Insert it above the company name and keep it small, about the height of two lines of text.
Use it in Supernatural
Supernatural: everything your business runs on, with Blue as your part-time admin. Word is one of its apps. In your workspace, you ask Blue for “a formal letter”. Blue starts from this template, writes in what you give it, and leaves [brackets] where something is missing. Before it makes the document, Blue shows you which template it chose and what it will write. Nothing is made until you say yes.
